Savages and helicopters

The aviation public discusses next accident MD600N which has occurred on October, 21st on Kuzbas. Circumstances of the incident are banal (Sunday – take off without the permission - complex weather conditions - has late seen an obstacle - has sharply started to brake - was hooked by a tail beam …), if not consider one fact: the pilot of the helicopter - Pavel Sudakov. He, on April, 20th, 2007 operated by another MD600N which has fallen near to Moscow ring road (MKAD) because of a stop of the engine owing to FULL (!!!) development of the fuel.

Six month later (!) and all have repeated again: one more senseless accident MD600N which has carried away four human lives.

When we discussed the first information from Kuzbas with one of the head of the State Research Institute of Civil Aviation (GOSNII GA), His reaction was instant and terrible - "savages"! Really "savages". What here conclusions? Surprise the regularity (the time in half a year) and the absolutely indifference to the helicopters failures and to its accidents in Russia. How many helicopters it is necessary to break and to ruin people that at last have paid attention on it inadmissible high level of breakdown susceptibility?

Airbus A320 crashed because of criminal showdown?

The Airbus A320 plane belonging to Armavia that was flying from Yerevan to Sochi and crashed on May 3 near Sochi coast was blasted, former pilot of the Armenian Airlines Vladimir Pogosyan has announced at a news conference on October 19.
According to him, the plane in good order could fall into the sea swiftly, for just five seconds, only after an explosion.
He put the blame for the A320 crash on management of Armavia and Zvartnots Airport, who gave permission for the plane to depart for Sochi, while a criminal showdown started in the airport before the departure and continued onboard. "I do not comprehend how the company and airport security services could allow those people to get onboard, knowing they could continue the showdown during the flight. Such negligence resulted in a tragedy. Four relatives of the crash victims, Russian citizens, say their relatives called them a few seconds before the crash and said there was a shootout onboard.
Besides, belongings with holes from bullets were found in the water on the spot," he said asking why such experienced pilots, who were the crew of the crashed A320, being aware of the risk of landing in Adler Airport, which is not equipped for bad weather conditions, nevertheless made such an attempt. "The thing is, very important persons were aboard, who made the crew to attempt landing. I can account for my words. Moreover, our pilots proposed to the parliament to establish a group to investigate causes of the crash, and we could have proved verity of our words," the pilot said, adding that commission was never established under pressure of some certain forces.
113 people were killed in the A320 crash, 105 of them passengers and eight crew members. The investigation has still continued, it has not given a clear answer on causes of the crash.

Pilots of crashed Airbus A320 did not want to land at any rate

No pressure was exerted upon pilots of Airbus A320 plane belonging to Armavia Airlines, crashed on May 3, 2006 near Sochi; Armavia Airlines Security Department head Arshak Nalbandyan is quoted by a REGNUM correspondent as stating on October 26 during discussions of reasons of the plane's crash in Yerevan.
According to him, available data demonstrates that no unauthorized persons were in the cockpit. Moreover, the crew's general mood testifies there was no tensed situation in the cabin, Nalbandyan informed. The decision to land was made in accordance with current rules; weather conditions corresponded with needed minimal requirements, he stressed, adding that the situation got out of hand only during the second attempt to land. Also, Arshak Nalbandyan disproved information on explosion aboard. "Having seen wreckage of the plane, I also believed there was an explosion aboard the plane; however, French experts have not revealed traces of explosion at the debris," he added. He stressed that the A320 plane had enough fuel to flight from Sochi to Moscow.
It is worth reminding; ex-pilot of Armavia Airlines Vladimir Pogosyan stated to the press that properly functioning plane might swiftly fall down within five seconds only if it exploded. He accused leaderships of the Armavia Airlines and Zvartnots Airport, who allowed the air liner to flight to Sochi while scuffle between two criminal clans continued aboard the plane, having started before the flight at the airport. Asking himself why experience pilots tried to land at Adler airport, which is not fit for landing under bad weather conditions, he stressed that 'VIPs' turned out to be aboard, who forced the crew to land.

IAC: A320 crash caused by inadequate actions of the captain

The Chief Department of Civil Aviation of Armenia has received the official findings of the Interstate Aviation Committee (IAC) concerning the crash of the Armenian Airbus A320 plane that fell into the Black Sea while flying from Yerevan to Sochi on May 3. The press service of the Department has informed REGNUM of the content of the findings:
"The investigation has shown:
With autopilot turned on, the plane was gliding strictly within the landing configuration and was already 300 meters high when the air controller instructed the crew to stop landing because of low clouds, and to make a climbing turn. When turning, the captain turned off the autopilot and, after climbing for a while, with improper bank and pitch attitude, began to land the aircraft - thereby, acting inadequately. Meanwhile, the second pilot failed to properly control the landing parameters (pitch, altitude, vertical speed). When the terrain warning system went off, the crew's actions were poorly coordinated and insufficient for pulling the plane up. No single system failed, the aircraft adequately reacted to both the instructions of the autopilot and the actions of the crew. There was enough fuel for safe landing."
The investigation was carried out by the IAC, a group comprising representatives of Russian Transport Control Service, Russian Aviation Department, Russian Air Navigation Department, the Armenian air authorities and experts from France (the A320 manufacturer-country) in compliance with the ICAO standards, the Intergovernmental Agreement of 12 regional states (including Russia and Armenia) and the air accident investigation rules. During the investigation the commissioners collected and examined all relevant data on the aircraft, the crew and the services controlling the flight, decoded and analyzed the data of the black boxes and the land air control services. With the help of special stands and trainers, a group of Russian, Armenian and French specialists simulated the whole flight, including the actions of the pilots.
To remind, the A320 fell into the Black Sea near Sochi, 6 km away from the shore, on May 3, 2006 02:13 AM. All the 8 crew members and 105 passengers onboard were killed.

Human error, fog to blame for Nepal WWF crash

KATHMANDU, Oct 21 (Reuters) - Human error and bad weather were responsible for last month's crash of a helicopter chartered by world conservation group WWF in which 24 people were killed, the chief investigator said on Saturday.
Junior forest minister Gopal Rai and diplomats were among those on board the Russian-made helicopter that crashed on Sept. 23 near Ghunsa village in Taplejung district, 300 km (190 miles) east of Kathmandu. There were no survivors. Keshari Raj Pandit, chief of the seven-member government-appointed investigation commission, said the pilot had failed to inform the nearest control tower before and after taking off from Ghunsa.
"The pilot does not appear to have fulfilled the mandatory flight rules," Pandit, who has already submitted his preliminary findings to the government, told Reuters. The rugged area is near Kanchenjunga, the world's third-highest peak at 8,586 metres (28,169 feet).
"The helicopter crashed in foggy weather into the hills, caught fire and then had a final stop at 4,033 metres (13,232 feet)," Pandit said, referring to the place where its wreckage was found.
"The flight data recorder shows that the machine was functioning until the last moment." The aircraft was carrying 17 Nepalis, two Russians, a Finnish diplomat, two Americans, a Canadian and a Swiss-Australian.
They had attended a ceremony to mark the handover of a conservation project supported by the WWF to a local community and were returning to Kathmandu.

False depressurization in air

More than hundred passengers who were onboard of the Boeing, have solved, that the interior is depressurized and have begun to dress the masks. Pilots had to decrease on the safe height - one and a half thousand meters. As have informed in the press-service of the airline, there were no the depressurization of passenger cabin, and oxygen masks have dropped out because of failure of automatics. Now on this fact check is spent. As a result of incident nobody has suffered, people who were onboard of the airplane have got off an easy fright.

The comment of ACRE' experts:
Whether false depressurization?
If there were no the depressurization of the cabin, what for the crew has made a decision on decrease to the safe height of 1500 meters? By the way, oxygen masks drop out automatically at pressure drop in a passenger cabin.
The statistics of MAK fixes 1-2 incidents in a month, connected with system of pressurization and air-conditioning on the airplanes of Boeing and Airbus, flying in Russia' sky.
Let's recollect, that the Cyprian Boeing was broke because of the depressurization …

EXPERTS HAVE RESTORED THE PICTURE OF ACCIDENT ÒÓ-154

The Technical Commission of Interstate Aviation Committee (MAK) on investigation of the accident of the plane Tu-154M "Pulkovo” Airlines ", broken on August, 22nd near Donetsk, has finished preliminary decoding of "black boxes". On the basis of the received data the experts could restore a picture of accident.
On these data, failure most likely became consequence of a combination of two major factors: inactivity of crew in the certain situation and complex weather condition. Also it is authentically established, that refusals of the system of the liner were not, writes Newsru.
According to decoding of negotiations of the pilot with the dispatcher, board RA-85185 has requested the sanction to climb to "390-th level" is about 11900 m. Because of malfunction of the auxiliary power unit (APU)of the plane Tu-134 of "Pulkovo" 23 journalists of the federal Russian and foreign editions cannot depart the second day from Naryan-Mar airport.
As the correspondent of "Interfax" who is being at the airport Naryan Mar has informed, the charter plane which has been organized by "Pulkovo" airlines for the visit of journalists in Yamalo-Nenetz District for the illumination of celebrating of "Day of a deer" and a course of the realization of the presidential national projects, it has appeared is faulty of the APU by means of which start of main engines is carried out has given up. Despite on numerous negotiations of organizers of press-visit with the representatives of "Pulkovo" airlines, any concrete measures is not accepted. The representatives of mass-media expect to leave of Naryan-Mar by the planes of the other airlines.
In the commercial department of "Pulkovo" airlines to the Interfax have informed, that "the combined situation with the faulty of APU' engine is regular. At its occurrence the basic engine is started by the ground power unit (GPU) which at the airport of Narya-Mar has not appeared". "We search for the ways under the decision of the developed situation", - have added in "Pulkovo".
ACRE' comments:
The given situation can be characterized as SHAMEFUL both for the maintenance service, and for the all management of "Pulkovo" airlines.
After the sign of the charter agreement to carry out (and for the considerable money) the charter flight to Naryan-Mar, "Pulkovo" airlines should send the alternative plane for delivery of its passengers back to Pulkovo. By this alternative aircraft the other APU and a technical brigade for replacement of faulty APU should be sent also. APU could be replaced within four hours. As a variant - to get GPU from any nearby airport, including military airports is possible also for the start of main engines of the Òó-134. Probably, "the brains" for this purpose in "Pulkovo" does not suffice, and it is a pity...
ACRE informs all the customers who have suffered in such situations, that ACRE' experts are ready to assist in technically competent registration of the claims to any carriers.

Air crash in eastern Congo kills 17: UN

KINSHASA (Reuters) - Seventeen people died in a plane crash in eastern Democratic Republic of Congo on Thursday, a United Nations spokeswoman said.
The Ukrainian-made Antonov 28 aircraft was flying in low cloud as it approached the airport at Bukavu in the South Kivu province when it clipped a mountainside and plunged into the forest in flames, the spokeswoman said.
All fourteen passengers and three crew were killed when the plane crashed some 15 km short of the airport runway, she said. "A rescue team arrived at the scene shortly afterwards but there were no survivors. The plane was completely burned," said Sylvie Van den Wildenberg, spokeswoman for the U.N. mission in Bukavu. The Antonov plane was operated by a small Congolese company. U.N. flights had been suspended due to poor weather conditions.
This accident was the latest in a spate of air crashes over the last year in the vast central African country, which held its first free elections in four decades on July 30 and is struggling to recover from years of war and chaos.
In June, the International Air Transport Association (IATA) called the air safety records of several African countries, including Congo, an "embarrassment" for the industry.
In March, the European Union banned all but one of the 51 carriers operating in Congo from flying in Europe. Following the criticism, the Congolese government has tried to improve air safety by grounding some airlines and regulating the amount of cargo that can be carried. But using old Soviet-made aircraft is often the only alternative for cash-strapped small African airlines. Such aircraft are extensively used in Congo, a country the size of Western Europe with only 500 km of paved roads.
